Who this helps

  • Adults and older adults noticing changes in memory, word-finding, or thinking in daily life.
  • Spouses, adult children, or caregivers concerned about a loved one’s memory, safety, or independence.
  • People with medical conditions that affect brain health (for example, stroke, seizures, concussion/TBI, or cancer treatment).
  • Individuals who need documentation of cognitive functioning for work, driving, benefits, or long-term care planning.

When to consider a memory & cognition evaluation

  • New or worsening forgetfulness that impacts daily routines, medications, or appointments.
  • Word-finding, organization, or judgment changes at home, work, or while driving.
  • Memory or thinking changes after concussion, stroke, COVID-19, or other medical treatment affecting cognition.
  • Uncertainty about whether changes reflect typical aging or something more concerning (for example, repeating questions or getting lost in familiar places).

What we assess

  • Memory (learning new information, recall, and recognition).
  • Attention, processing speed, and mental flexibility.
  • Language and word-finding, visuospatial and navigation skills.
  • Executive functions and everyday functional skills (for example, managing medications, finances, and routines).

Memory & dementia evaluation FAQs

Do you evaluate both older adults and younger adults with memory concerns?

Yes. We provide memory and dementia evaluations for adults across the lifespan, including seniors. When appropriate, we invite a spouse, adult child, or other caregiver to share observations and attend intake or feedback so everyone understands the plan.

Can this evaluation tell us whether changes are dementia or normal aging?

The evaluation shows how memory and other thinking skills are functioning compared with age peers. We integrate test results with history and medical information to help your primary care provider or neurologist determine whether changes fit typical aging, mild cognitive impairment, dementia, or another cause.

What should we bring to a memory and dementia evaluation?

Please bring glasses or hearing aids, a current medication list, and any prior testing or brain imaging reports you want reviewed. A family member who knows the person well is often helpful, especially if they can describe changes in day-to-day functioning and safety.

What do the results include?

You receive a clear written report summarizing test findings, diagnoses if applicable, and practical recommendations for safety, independence, work, driving, and daily routines. With your permission, we can share the report with your primary care provider, neurologist, or other members of your care team.
